File UploadsHaving Trouble Uploading Files? We’re Here to Help
We’re aware that some users may experience issues with file uploads when submitting an application to surrender a dog, foster, or adopt through our website.
This can happen due to browser settings, device compatibility, internet connections, or file size or file type restrictions. We know how frustrating this can be, especially when you’re trying to help a dog, and we truly appreciate your patience.
To make sure your application is received without delay, we’ve created this alternative upload page. If you had trouble uploading photos or documents, please use the form below to securely submit your files.
Our team will manually attach your uploads to your application so nothing is missed.
If you’ve already submitted your application but couldn’t upload files, you do not need to resubmit the entire application—just upload your files here.
Please upload the files requested for your application below and select the section that matches the form you completed: Surrender, Foster or Adoption
Accepted file types:
Photos: .AVIF, .BMP, .HEIC, .HEIF, .JPEG, .JPG, .PNG, .WEBM, .WEBP

If you experience issues uploading files, please try again using smaller file sizes. If you need help submitting images or documents, you may text or leave a voicemail at (801) 432-0785, and we will return your call as soon as possible.
Surrender
Please upload at least one clear photo of the dog you are surrendering. If possible, include a front, head-on view and a standing side view to help us better assess the dog.
Dog Medical Records – Upload any veterinary records you have, including vaccination history and medical notes. Most veterinary offices can email these records as a PDF, and photos of records are also accepted. Providing complete records helps us ensure the best possible care.
Foster – Virtual Home Check
Inside of Home – Upload photos showing where the dog will live, including main living areas, the sleeping area, and any safety features such as baby gates or ramps. We look for a clean, well-kept home with a designated dog space, access to water, and reasonable safety accommodations. Baby gates and ramps are easy to add and may be available to borrow.
Outside of Home – Upload photos of your outdoor space, including the yard, patio, or garden, dog doors if applicable, and the complete fence and gate enclosing the yard. For apartments or townhomes, please include nearby green space available for the dog. We look for a secure outdoor area free of major hazards, with fencing that has no holes or gaps and minimal debris.
Adoption – Home Photos
Please upload photos of both the inside of your home and your outdoor space. When reviewing submissions, we look for basic safety measures such as baby gates at stairs, a secure yard, no firearms visible, and a safe, comfortable space for the dog including a bed or ramps.
