Waiver and Release

This Hold Harmless & Media Release Agreement (the “Agreement”) is made as of (“Effective Date”) by and between Rocky Mountain Dachshund Rescue (“RMDR”), located at PO Box 522461, Salt Lake City, Utah 84152, and (“Participant”), located at (Enter your legal address below).

1. Assumption of Risk & Hold Harmless

I understand that participating in activities with Rocky Mountain Dachshund Rescue (“RMDR”) — including adoption meet-and-greets, transporting dogs, fostering, adopting, volunteering, attending events, and interacting with dogs — involves inherent risks. These risks may include, but are not limited to, dog bites, scratches, illness, allergic reactions, property damage, accidents during transport, and other unforeseen events.

I voluntarily assume all such risks, whether known or unknown, and accept full responsibility for any injury, loss, or damage that may occur to me, my family, or my property as a result of participation in these activities.

I agree to release, indemnify, and hold harmless RMDR, its officers, employees, and volunteers from any and all claims, demands, liabilities, damages, costs, or expenses of any kind arising out of or connected with my participation in these activities, to the fullest extent permitted by law.

2. Photo & Media Release

I consent to RMDR using my likeness, and/or the likeness of my family members or minor children, as well as photos of my pets, adopted dog(s), in photographs or videos taken during RMDR events, home visits, or those I voluntarily provide.

I grant RMDR the unrestricted, worldwide, and perpetual right to use, publish, edit, or distribute these images in any media for promotion, education, or fundraising, without compensation.

I release RMDR, its volunteers, and affiliates from any claims or liabilities arising from such use. If minors are depicted, I confirm that I am their parent/guardian and authorized to give this consent.

3. General Terms

I am at least 18 years of age and have full legal authority to sign.

This agreement is governed by Utah law, and disputes will be resolved in Utah courts.

If any part of this agreement is found invalid, the rest remains enforceable.
